The Registration Office is open Monday through Friday from 8 a.m. to 4 p.m.
The Registration Office is responsible for:
- Student Registration
- Enrollment Verification
- Residency and Custody Issues
- Demographic Updates
- Annual School Census
Registering a new student
All registration documents must be downloaded, filled out and printed. Please note that this packet must be printed single-sided. Most of the documents can be filled out and saved electronically before printing, however, the HIPPA form and health appraisal forms must be filled out after printing. Once you have completed, signed and dated all of the forms, bring the registration packet to the Registration office, 139 Fairbanks Road, Churchville, door 26. Be sure to provide all required documentation listed below. Failure to do so will delay the registration process. A parent's photo ID is required to complete registration.
In order to register a child for school, the biological parent or court-appointed legal guardian must have legal residence in the Churchville-Chili District. It is illegal for children to attend school where the parent or legal guardian does not reside.
Copies of the following documents are necessary for registration of a new student and must accompany the registration documents:
- Original Birth Certificate (will make a copy)
- Immunization Record
- Proof of Residency
- Parent/Guardian Photo ID
- Custody or Guardianship Legal Documents (if applicable)
Please refer to the welcome letter (page 2) of the Registration Packet for specifics on acceptable documents. Please note that this packet must be printed single-sided.
Parents should receive registration forms along with a letter notifying you of the dates of registration for your child's elementary school in February for the fall of that specific year. Children who will be 5 years old on or before December 1, of that calendar year are eligible. If you do not receive a letter or registration packet by mid-February, or are not sure which elementary school your child will attend in the fall, please feel free to contact the Registrar at the number above.
Open Registration takes place in the spring at the elementary school that your child will be attending. Dates will be posted on the online district calendar page of this website.
Moved Within the District
If you have recently moved within the district you will need to contact the Registration Office to change your student’s information. You will need to fill out a change of address form (below) and provide the Registrar with proof of residency (as explained above). The Registration office is located at 139 Fairbanks Road (attached to the Middle School) and can be reached by calling 293-1800 extension 2041 or emailing firstname.lastname@example.org.
Due to HIPAA Regulations, medical records cannot be released without the authorization of the patient or the patient's parent/guardian. For enrollment eligibility purposes, it may be necessary for the District to request immunization records, health records or other medical information. A Medical Release Form can be found at the bottom of this page.
Throughout the course of the school year, there may be opportunity to publicize student accomplishments. This may occur in the form of photographs or videos with names and a written summary in the district or building newsletters, website, social media platforms, television, radio or local newspapers. In order to exclude your child from participating, please submit a letter to the principal in your child(ren)’s building.
