The Registration Office is open Monday through Friday from 8:00 a.m. to 4:00 p.m. (Summer hours are 7:30 a.m. to 3:00 p.m.)

The Registration Office is responsible for:

  • Student Registration
  • Enrollment Verification
  • Student Information Systems Management and Reporting
  • District Records Management
  • Residency and Custody Issues
  • Demographic Updates
  • Annual School Census

Registering a new student: Please refer to the menu on the left to find grade specific registration forms.  

Download, print and fill out the registration documents listed. Once you have completed all of the forms you can drop the registration packet off in person to the Registration office or mail it to: Churchville-Chili Central School District, attn.: Registration Office, 139 Fairbanks Road, Churchville, NY 14559.  Be sure to provide all required documentation listed below.  Failure to do so will delay the registration process.

 

In order to register a child for school, the biological parent or court-appointed legal guardian must have legal residence in the Churchville-Chili District. It is illegal for children to attend school where the parent or legal guardian does not reside. 

 

 

Copies of the following documents are necessary for registration of a new student and must accompany the registration documents:

  • Birth Certificate
  • Immunization Record
  • Proof of Residency
  • Parent/Guardian Photo ID
  • Custody or Guardianship Legal Documents (if applicable)

 

The following are accepted proofs of residency:

 

·         For Homeowners:                                                         

o    A mortgage statement or                                               

o    School, Town or County Tax Bill or                                    

o    Homeowner’s insurance statement or                                    

o    Purchase papers for a home

 

·         For those renting or leasing a house/duplex/apartment:          

o    Current lease or rental agreement. This must be in effect at the time of registration.                                   

o    If you are on month to month or have no current lease then you must provide the following:            

o    A notarized statement from the landlord that the parent/guardian and the children are residing at the property, including landlord’s phone number AND                 

o    Proof of homeownership from the landlord

 

·         If you are not the primary owner or renter of the property:     

o    Request from the landlord that your name be put on a new lease and provide that as your proof of residency or:                                                  

o    Complete the School District’s Dual Residency Affidavit.  The document must be signed by the primary owner/renter and yourself.  Signatures must be notarized.  Additionally the primary owner/renter must provide proof of residency(as mentioned above). You must also prove you live there.  Valid proof of residency for you include Bank Statement, Major Credit Card bill, Phone bill, payroll stub, valid NYS Driver’s license.  You must provide 3 of these for yourself.

 

Kindergarten:

 

Parents should receive registration forms along with a letter notifying you of the dates of registration for your child's elementary school in February, 2013. Children who will be 5 years old by December 1, 2013, are eligible to register. If you do not received a letter or registration packet by February 9, or are not sure which elementary school your child will attend, please feel free to contact Registrar, Geri Pehta, at the number above.  

 

Open Registration takes place in the spring at the elementary school that your child will be attending.

 

Moved Within the District:

                                              

If you have recently moved within the district you will need to contact the Registration Office to change your student’s information. You will need to fill out a change of address form and provide the Registrar with proof of residency (as explained above). The Registration office is located at 139 Fairbanks Road (attached to the Middle School) and can be reached by calling 293-1800 extension 2041.

 

 

Medical Regulations:

Due to HIPAA Regulations, medical records cannot be released without the authorization of the patient or the patient's parent/guardian. For enrollment eligibility purposes, it may be necessary for the District to request immunization records, health records or other medical information. A Medical Release Form can be found at the bottom of this page.
